A cutaway image of a tree being planted gives several tree planting pointers. There is a loosened area of soil 2-5 times wider than the diameter of the root ball. The root ball itself rests on solid, undisturbed soil, and the top of the root ball sits at grade level. After planting, mulch the entire loosened area, except right around the trunk. Create a small dam 3 feet in diameter around the trunk, to hold in water and let it percolate into the soil.

This keeps the bark from tearing. The second image shows a second cut, all the way through the branch, 2 to 3 inches above the first cut.
